SDA Resident Survey 2.0

SDA Services would like to find out how living in Specialist Disability Accommodation (SDA) has made an impact on people’s lives.

We would like people who have lived in SDA for 6 months or more to complete a survey. The survey asks about people’s lives and supports before and after living in SDA.

After completing the survey you can enter a prize draw to win one of 5 x $100 Freedom vouchers.

About the Survey

The SDA Resident Survey was developed with an independent company, Shared Direction Consulting.

Who can complete the survey?

You can complete the survey if you are an NDIS participant who has lived in SDA for 6 months or longer.

It is up to you if you want to complete the survey. You can start or stop at any time.

Tips for completing the survey:

• You can ask someone you trust to help you.

• Have your NDIS plan handy to answer questions about your SDA type and support hours in SDA.

• You can do the survey online or if you need phone assistance contact Amanda Dodds on 1300 001 003 or info@sdaservices.com.au.

• The survey will take around 15 minutes but could be longer depending on your answers. If you want a break, copy the URL (web address) and send it to yourself so you can go back later.

Privacy

We only collect personal information you choose to provide in the survey, either because you want us to call you, or you want to enter the prize draw. The information you give us will be kept private and confidential. No individuals will be able to be identified in the results. It will be reported as a group. Only the research teams will have access to the data. This confidence can only be broken if required by law. Once the study is completed, the data collected from you will be de-identified and stored securely at SDA Services.
